Advance Directive Workshops

Description

DeKalb Township is making it easy to start the new year off on the right foot by hosting Advance Directive Workshops every Monday & Friday in January, featuring Northwestern Medicine's Doctor of Nursing, Annette Meares. 

Come learn about Living Wills, Powers of Attorney for Healthcare, & POLST Forms. Complete your forms and walk out of the workshop with your executed documents in-hand, and a smile on your face. Workshops begin at 1 PM, 2 PM, & 3 PM on January 16th! 

This FREE offer makes it easy to complete your advance directives without the need to hire an attorney. Anyone over the age of 18 can participate. Simply give us a call at 815-758-8282 to sign-up.
